Who Does What, and How to Choose the Right Fit for You
When you’re ready to seek mental health support, knowing who to turn to can feel confusing. In Ontario, several regulated professionals can help with mental health concerns—but their roles, training, and scope of practice vary.
This guide will help you understand the key differences between Social Workers, Psychotherapists, Psychologists, Psychiatrists, and other professionals offering care in Ontario. Whether you're looking for talk therapy, diagnosis, medication, or a combination of these, this post will help you make an informed decision based on your needs.
1. Registered Social Workers (RSWs)
Regulated by: Ontario College of Social Workers and Social Service Workers (OCSWSSW)
Can provide: Talk therapy, psychosocial assessments, EAP counselling, systems-based support
Cannot: Diagnose mental disorders or prescribe medication
Registered Social Workers provide therapy and support that considers the whole person in their environment. Their approach is rooted in a holistic understanding of mental health, drawing from individual experiences, family systems, social identity, trauma, oppression, and systemic influences.
RSWs can:
Provide individual, couples, or family therapy
Support with grief, trauma, anxiety, burnout, parenting, identity, life transitions, and more
Offer services through Employee Assistance Programs (EAP) or private practice
Work in schools, hospitals, agencies, or independently
Often covered under extended health insurance plans
RSWs cannot diagnose mental illnesses or prescribe medication, but they are often the first point of therapeutic support for many individuals in Ontario. Wait times in private practice are often short, and many RSWs offer virtual therapy.
2. Registered Psychotherapists (RPs)
Regulated by: College of Registered Psychotherapists of Ontario (CRPO)
Can provide: Psychotherapy using approved modalities
Cannot: Diagnose mental disorders or prescribe medication
Registered Psychotherapists focus exclusively on providing psychotherapy. They are trained in various therapeutic approaches such as Cognitive Behavioural Therapy (CBT), Emotion-Focused Therapy (EFT), Mindfulness-Based Interventions, and more.
RPs can:
Offer talk therapy for a range of mental health and emotional issues
Specialize in areas like trauma, relationships, anxiety, or depression
Work in private practice, community organizations, or health centres
Coverage for RP services varies by insurance plan. Some plans now include Registered Psychotherapists, but not all do. Check with your provider before booking. Like RSWs, RPs typically offer accessible, flexible, and compassionate support.
3. Psychologists
Regulated by: College of Psychologists of Ontario (CPO)
Can provide: Psychological assessments, diagnoses, psychotherapy
Cannot: Prescribe medication
Psychologists have advanced doctoral-level training (PhD or PsyD) in psychology. They are highly skilled in diagnosing mental health disorders and conducting in-depth psychological testing.
Psychologists can:
Diagnose conditions such as ADHD, depression, anxiety, PTSD, and learning disabilities
Provide evidence-based therapies (e.g., CBT, DBT, EMDR)
Conduct psychoeducational and neuropsychological assessments (often needed for schools or legal purposes)
They must declare a specific scope of practice (e.g., clinical psychology, counselling psychology) and cannot treat outside of that scope. Services are not covered by OHIP but are often included in extended benefits. Wait times can be longer depending on availability and specialization.
4. Psychiatrists
Regulated by: College of Physicians and Surgeons of Ontario (CPSO)
Can provide: Medical diagnosis, medication, psychotherapy
Can: Prescribe medication
Psychiatrists are medical doctors with a specialty in mental health. They are the only mental health professionals (besides family doctors) who can diagnose and prescribe medication for psychiatric conditions.
Psychiatrists can:
Diagnose and treat complex or severe mental illnesses (e.g., schizophrenia, bipolar disorder, major depression)
Prescribe and manage psychiatric medications
Provide psychotherapy, though many focus primarily on medication management
Collaborate with therapists and social workers for comprehensive care
Psychiatry services are covered by OHIP, but referrals are required through a family doctor or nurse practitioner. Wait times can be several months or longer depending on urgency and location.
5. General Physicians (Family Doctors)
Regulated by: College of Physicians and Surgeons of Ontario
Can provide: Initial screening, medication, referrals
Limited in: Ongoing mental health therapy
Family doctors are often the first step when seeking mental health care. They can:
Rule out medical causes for symptoms (e.g., thyroid issues, vitamin deficiencies)
Prescribe medications such as antidepressants or anti-anxiety medication
Refer you to Psychiatrists, Psychologists, or Social Workers
While some physicians have additional training in mental health, their time is limited and they may not offer regular therapy. Still, they are a crucial part of the mental health care system and often serve as gatekeepers for referrals.
6. Counsellors (Unregulated Title)
Note: In Ontario, the title "Counsellor" is not regulated, meaning anyone can technically use the title regardless of training. This is different from Registered Psychotherapists who are regulated under the CRPO.
Some Counsellors may have high-quality training and belong to voluntary associations such as:
Canadian Professional Counsellors Association (CPCA)
Canadian College of Professional Counsellors and Psychotherapists (CCPCP)
However, unless they are registered with CRPO or another regulatory body (e.g., OCSWSSW), their title does not guarantee professional standards, supervision, or accountability. Always verify credentials before starting therapy.
